How Do I File for Unemployment Insurance?

You will need the following information to file:

  • Your Social Security Number.
  • The names and dates of employment of ALL your employers for the past 18 months.
  • The name and local number of your union hall, if you obtain work through a union.
  • Your Utah Driver's License or Utah Identification Card number if you have one.
  • Your Alien Registration Number if you are not a US citizen.

When calling to file a new claim:

(888) 848-0688 Remainder of State and Out of State

Please have the following information available:

  • Social Security Number
  • Alien Registration Number (for non-U.S. citizens)
  • Utah Driver’s License Number or Utah ID Number (if you have one)
    Names and addresses of all employers that you worked for in the last 18 months
  • Personal Identification Number (PIN), which you select
    Mothers maiden name or another code word
  • Pencil and paper for note taking

If you do not have this information available please gather it before continuing to file your claim. This site is available from 2:00 AM to 9:00 PM, seven days a week. The filing process takes, on average, 30 minutes to complete. Please allow enough time to complete the process. If you attempt to submit a page after 9:00 PM all your information will be lost.
